Terms and conditions for the Quit Stop Wirral free prize draws

  1. The free prize draws will run from 1 April 2011 to 31 March 2012.
  2. 2 Prize draws will be drawn monthly May 2011 – August 2011 and then Bi monthly ( every other month) Sep 11 – March 12 .Draws will take place at the end of the month followed by presentations the following month. The top prize will be £100 and the remainder £50. All unclaimed prizes will be returned to the prize draw fund and drawn at the next prize draw occasion
  3. To enter any of the free prize draws you must live in Wirral and have a registered Wirral postcode or, be registered with a Wirral GP. You must be 18 years or over.
  4. To qualify for the quarterly and monthly prize draws you must be a smoker at the point of registration and be 4 weeks smoke free to enter into the draw. The prizes will only be awarded directly to the winners upon taking a carbon monoxide test and signing a declaration that they had been a smoker at the start of the 4 week period.
  5. Non smokers are not allowed to register for the monthly and quarterly free prize draws.
  6. If you are employed by Wirral Community NHS Trust, Wirral News or Asda you will not be eligible to win the Asda vouchers.
  7. All winners will be selected at random.
  8. All the Quit Stop Wirral free prize draw winners will be notified within 28 days of the prize draw taking place. Winners will be contacted by telephone and letter.
  9. Wirral Community NHS Trust will detail the names of winners on the Quit Stop website. The winners name and the area in which they live will be profiled, eg John Doe, Tranmere.
  10. Entries will be made automatically from registrations held on the service database of those clients smokefree at 4 weeks.
  11. The winners may be requested to take part in any publicity in connection with the draw, which may include photos of winners receiving their prize.
  12. No cash or other alternative prizes will be available unless otherwise agreed in writing by the promoter.
  13. Gift cards are not transferable and will only be awarded to the registered person.
  14. If there are not enough entries into any of the prize draws, the promoter has the right to cancel that individual draw.
  15. If you change your contact details it is your responsibility to notify the promoter.
